This list of payors and health plans with which Glenwood Surgery Center participates is not all-inclusive.  Glenwood Surgery Center will make every effort to accept all patients with any payor or health plan not on this list as an out-of-network provider. Please contact facility for special arrangements.

SELF PAY

You will be contacted prior to your surgery with an estimated procedure cost for you surgery.  A down payment equal to 60% of the total estimated amount due is expected.  You will be asked to complete a financial agreement.  The remaining balance will be due within 90 days from your date of service. 

SELF PAY – COSMETIC SURGERY  - ELECTIVE SURGERY

Payment in full must be received on the day of surgery.

If your insurance company is not listed, it may be considered to be part of one of the networks listed above.  Please call our office at 951-688-7270 and ask to talk with the billing department for more information.
